Application Process Changes due to COVID-19:

 

            Due to widespread health concerns regarding COVID-19 (Coronavirus), we are making changes to the application process. Please read the following information carefully.

 

  1. Download and complete the application form for your desired course (EMT or Advanced EMT).

  2. Scan and email the completed application form to us at instructors@northstar-ems.us.

  3. Once we have received your completed application form via email, you will receive an orientation packet in a .PDF file.

  4. Print, fill out completely, and sign all forms in the orientation packet.

  5. You must bring ALL completed forms AND payment to Kim McClain at our main office (2106 17th Ave., Tuscaloosa, AL 35401) no later than the application deadline of April 27th, 2020.

  6. Completed application packets and payment will be accepted Monday-Friday from 9:00 am until 3:00 pm.

  7. For those choosing to take advantage of the payment plan option, your initial payment will be due with your completed orientation packet, and your payment agreement contract will be completed and signed at that time.

 

Please note that the dates/nights for the class could be subject to change depending on circumstances moving forward. Please monitor your email and our website for any future updates.
